Problem Plugin Name And Version
PHP And MYSQL Version
Possible Conflicting Plugins (Optional)

Before You Post:

DO A SEARCH to see if someone has posted the problem before and whether it has been solved.
SWITCH to WordPress Default Theme to see if the plugin works.
DEACTIVATE ALL plugins except the problematic plugin to see if there is a pluin conflict.
