Theme Name
Blog URL
Problem Plugin Name And Version
PHP And MYSQL Version
Possible Conflicting Plugins (Optional)

Before You Post:

DO A SEARCH to see if someone has posted the problem before and whether it has been solved.
SWITCH to WordPress Default Theme to see if the plugin works.
DEACTIVATE ALL plugins except the problematic plugin to see if there is a pluin conflict.

Note:

Plugin conflicts SELDOM get solved.
We DO NOT provide support for CSS UNLESS the CSS is preventing the plugin from working normally.
We DO NOT provide support for any version of prior to WordPress 2.7.
